Build the review:
1. **Themes first**: distill my notes into 2-3 strength themes and 1-2 growth themes. A review with ten scattered points lands as noise; three evidenced themes land as signal. If my notes only support recent events, tell me (recency bias is the #1 review failure), and ask for one example from the first half of the period.
2. **Evidence in SBI form**: for each theme, 1-2 examples written as: Situation (when/where, specific), Behavior (only what a camera would record; "interrupted three times in the client call", never "was dismissive"), Impact (effect on team, customer, or number). Rewrite any interpretation in my notes into observable behavior, and flag where I've given you a judgment with no incident behind it: [EVIDENCE NEEDED].
3. **Goals scorecard**: last cycle's goals vs. what happened, each with a number or a plain "not met + why", no spin in either direction.
4. **Growth areas, forward-looking**: each growth theme gets: the behavior to change, what good looks like in 90 days, and a concrete support action ([FOR MANAGER REVIEW: training, pairing, scope change]). For self-assessments: name the gap yourself with mitigation already started, self-identified beats manager-identified every time.
5. **The summary paragraph**: the whole review in 4-5 sentences, balanced, no vague amplifiers ("great team player", "rockstar", "needs to be more strategic" are banned: each must be replaced by its underlying behavior).

Rules: calibrate to the rubric language I provided. Nothing enters the review that isn't in my notes: a review is testimony, not creative writing. Where my notes are one-sided (all wins or all misses), say so and ask the balancing question before finalizing.

How to use

SBI (Situation-Behavior-Impact) is the Center for Creative Leadership's feedback structure, and its camera test (write only what a video would show) is what keeps reviews fair and defensible. The recency-bias check matters more than it seems: reviews written from memory over-weight the last six weeks. Keep a running notes file all cycle and this prompt turns review season into an hour.
